Eligibility: For free and low-cost breast and cervical cancer screenings, the applicant must be a woman age 50-64, must not be insured, and must have an income at or below 200% of the Federal Poverty Level.
Service includes: Breast and cervical cancer screening exams, mammograms, Pap smears, and clinical breast exams.

Eligibility: This program provides health insurance for uninsured children from birth to age 18. This includes the following services: Doctor visits, checkups, immunizations, hospital services, surgery, prescriptions, emergencies, vision, hearing, dental, and mental health services.
Income eligibility requirements: Must be a U.S. citizen or qualified non-citizen (contact program for information), may not be eligible for Medicaid, may not be a dependent of a state employee eligible for health insurance, and must not be in a public institution.
Acceptable forms of income verification: A families most recent tax return (form 1040), your wage and earning statement (w-2 form), current pay stubs (covering the last four weeks) that show Gross Income amount, the cost to add your child or children to health insurance coverage if your employer offers it, your childrens social security numbers or the date applied for if you have not received the social security card.

Eligibility: Applicant must be a U.S. citizen or qualified alien who resides in this state and has low income, has limited resources, and is included in one of the following categories: Is age 65 or over. A family with dependent children. Pregnant women. Children under age 19. Disabled adults or have permanent kidney failure.
Service includes: Quality health care to eligible low-income individuals, children, families with children, and pregnant women. Also provides comprehensive medical care, including, but not limited to, physicians, hospitals, lab and x-ray services etc. This is the original Medicaid program. There is no cost for those who meet income guidelines, however, co-pays may apply for some services.
